To save time, you can first create user online on the government passport website. There you can upload the basic details and the information needed. You would have two options; Tatkal or normal and also an option to apply for interview in nearby passport kendra.
Are both Parents needed at Passport Seva Kendra(PSK)
Yes, both the parents are required at the PSK if applying for the passport of their minor child. If there is any urgency or both cannot be present at the same time, one parent should be available with the passport of both the parents.
Annexure-D should also be carried which should have the signature of parents.

What is Annexure-D for Minor Passport?
Annexure-D is a form which is filled by the parents of the Minor for whom applying for passport. It is a declaration of Parent/Guardian for minor passports.
Types of Annexure for Minor Passports
Annexure “C”: Declaration of Parent/Guardian for Minor Passports (one parent not given consent).
Annexure “D”: Declaration of Parent/Guardian for Minor Passports (Annexure “D”).
Annexure “F”: Specimen Affidavit for a passport in lieu of lost/damaged passport.

- Step 1: Register with details of minor on the Passport Seva Website
- Step 2: Create a login ID and password for the minor
- Step 3: Save the username and password for your record
- Step 4: Fill in the application form
- Step 5: Once you have submitted the application form successfully, you can make the payment
- Step 6: Search for the nearby Passport Seva Kendra for interview

Documents needed in Passport Seva Kendra
- Date of birth certificate of your child
- Your passport photo copy of four pages; 1st second, second last and last
- Aadhaar Card details or ID proof of your child. Make sure the date of birth and parents name are same as yours on the documents.
- Most importantly filled Annexure D form.
- You can also carry photos and other ID proof copies but above are the required ones.
- Keep extra copies for requirement.
